FORTITUDE DATA PROTECTION AND CONFIDENTIALITY POLICY
 
FORTITUDE will collect and use/process your personal information in the manner described below.

B) CONSENT/CHOICE STATEMENT
FORTITUDE  does not sell or rent personal information collected from you to third parties.
 
You explicitly specifically consent to FORTITUDE  and its authorized representatives to use the data for the purposes mentioned in paragraph I above.
 
You acknowledge that your personal information may be transmitted and used/processed by FORTITUDE  and its authorized representatives entities outside of your home country/jurisdiction and explicitly specifically consent that your personal information can be transmitted stored and processed in such third countries for the purposes specified in paragraph I above.
 
C) DATA QUALITY AND SECURITY STATEMENT
 
FORTITUDE uses security measures in order to protect the data we have under our control against manipulation, loss, destruction, access by unauthorized persons, misuse, alteration or against unauthorized disclosure. You take on the personal responsibility of ensuring that your password is kept confidential. FORTITUDE will not be liable for the manipulation, loss, destruction, unauthorized access, misuse, alteration, or unauthorized disclosure of your personal information due to negligence or failure on your part to ensure the confidentiality of your password.
 
Our security procedures are continually enhanced as new technology becomes available.
 
FORTITUDE does not sell or rent personal information collected from you to any third parties.
 
We would like to use your data for the purposes specified above. Naturally your participation in such collection of personal information for the specified purpose is voluntary. Should you not consent, you can inform us at any time, so that we can prohibit the use of your data accordingly.

Release of Liability *:

FORTITUDE RELEASE OF LIABILITY
 
I acknowledge that the above information is factually correct. I therefore agree to participating in the activities of the FORTITUDE held on all locations, and the use of all FORTITUDE sports equipment.
 
I agree that I will not hold the Coaches of the FORTITUDE or the site liable for any losses, liabilities and damages, injury or death suffered or incurred by myself whilst taking part in any of the activities run by FORTITUDE.
 
FORTITUDE MEDIA CONSENT
 
* Photographs and / or videos may be taken of myself taking part in sporting activities for promotion of the club and displayed on FORTITUDE website, social media, press releases, articles, and in newsletters in accordance with FORTITUDE safeguarding guidelines.

Refund Policy *:

FORTITUDE REFUND POLICIES
 
Fees are non-refundable unless it is due to extenuating circumstances that is medically related. Each case will be assessed on its merit with the provision of the necessary documentation.
    
An administration fee of QAR 250 will be applicable. 
    
Refunds will be processed within 3 weeks from the date of approval. 
    
We reserve the right to deliver our programmes online if any government restrictions relating to the COVID-19 pandemic prevent the physical delivery of programmes. No fees will be returned in such instances.

COVID-19 Declaration Form *:

FORTITUDE COVID-19 DECLARATION (ADULTS 18+)
 

Following the latest Qatar government guidance for the lifting of restrictions for COVID-19, we will be reopening FORTITUDE with a number of precautionary measures in place.  We are working on the assumption all members will have completed the quarantine requirements, if travelled internationally, as per Qatari law by the time they return to training.

Each morning, before training with FORTITUDE we need you to complete the following steps.

* Make sure you do not have a sore throat or other signs of illness, like a cough, shortness of breath, difficulty breathing, fatigue, muscle or body aches, congestion or runny nose, diarrhoea, severe headache or vomiting.  If you display any of these symptoms, you must stay at home and inform your coach. 

* Take your temperature, each morning before training. If the temperature is 37.8°C or higher, you must stay at home and inform your coach. 
    
* If you have had close contact with a COVID-19 case, you must stay at home and inform your coach.

Once you are in our facility, your temperature will be taken by the security team and then your coach.  If your temperature is above 37.8°C, then you’ll be asked to leave the premises immediately.

You must have received 2 doses of the COVID-19 vaccination in order to take part in the FORTITUDE programme as per the Ministry of Public Health guidelines.

Your Ehteraz app status will be checked upon entry to the facility. Anyone who does not have a green status also displaying the golden border and vaccination stamp will be denied entry to any of our facility. 

We request that you work with us in ensuring the health and safety of all members and staff at FORTITUDE and for this your cooperation is required.
